I am an in-network provider with:

Co-payments or deductibles are due at the time of service.

If you utilize an insurance carrier other than these, the practice is happy to assist you by providing a coded receipt that you may submit to your insurance company to seek out-of-network reimbursement. Many insurance companies have out-of-network benefits that reimburse a percentage of the session cost.

If you are interested in exploring your out-of-network benefits, please call your insurance company directly and ask them the following:

  • Do I have out-of-network mental health insurance benefits? Some companies call these behavioral health benefits. 

  • What is my deductible, and has it been met?

  • How many sessions per year does my health insurance cover?

  • What is the coverage amount per therapy session?

  • Is approval required from my primary care physician?

  • Is teletherapy a covered service? (video sessions)


Counseling Fees

Self-pay options without insurance constraints are available for those seeking therapy. With transparent rates and personalized care, your mental well-being is prioritized. Contact the office to discuss out-of-pocket rates and start your path to healing.

All major credit cards are accepted as well as checks, credit HSA/FSA cards, and cash.


Cancellation Policy

Your session time is valuable and reserved solely for you. Cancellation of your appointment is required at least 24 hours in advance unless there is a medical or family emergency so that someone else can be supported. Otherwise, you will be required to pay the agreed-upon $65.00 late cancellation fee. 

 


Forms

Intake paperwork will be completed via a secure online patient portal that will be emailed to you following the scheduling of your initial session. All forms must be completed before the initial session can occur.  If computer access is limited, please alert the office.  We will explore alternative ways to complete the required paperwork. Initial paperwork will include a demographic form, medical and psychological background forms, psychological assessments, informed consent documents, credit card information forms, and privacy practices disclosure forms.
